Blake Medical Center’s Diabetes Self-Management Education Program


Blake Medical Center’s Diabetes Self-Management Education Program is the only ADA recognized program in Manatee County.  The program, for patients with newly diagnosed or “out-of-control” diabetes, is held monthly.  Classes are Wednesday, Thursday, and Friday mornings (a detailed schedule and description of the curriculum is available in the Food and Nutrition Services Department).  A local physician’s referral is required for program admission.

Licensed professionals including two Certified Diabetes Educators teach the comprehensive, 10-hour program.  Additionally, lunch is provided on Wednesday and Thursday and each attendee is allowed to bring a spouse, significant other, or other support person with them to the classes at no additional cost.

Medicare covers the cost of the service for patients who meet the Medicare guidelines.  With appropriate authorization, other insurance companies can also be billed.

When a person has completed the Diabetes Self-Management Program, the referring physician is sent a summary of the curriculum points taught, chart notes, and results of the blood glucose self-monitoring done during the class sessions.
